Care protocol for erectile dysfunction

ABSTRACT

Erectile dysfunction (ED) is the most frequent consulting reason in Clinical Sexology Services in Cuba. Therefore, the design of a care protocol to homogenize its multidisciplinary approach, through an applied descriptive research, was decided. The aim was to provide the Cuban scientific method of clinical care for ED for all three levels of primary, secondary and tertiary health care. The literature of the last five years in Scielo, Clinical Key and Pubmed databases was reviewed. The Manuals of Good Practices of Colombia, Spain and Europe were obtained. The clinical experience of the authors was incorporated. The established therapeutic recommendations were: a) at the primary level (specialists of Integral General Medicine): comprehensive sexuality education focused on the modifications of the sexual response in the different stages of life, preventing, identifying and controlling risk factors; sexual counseling with the phallocentric deconstruction of eroticism; and the indication of sildenafil for ED caused by compensated chronic medical illnesses; b) if great anxiety or couple conflicts is detected, the case should be referred to the municipal sexual health group where they are able to use sexual and couple therapy; c) if there is no improvement, it must be referred to the provincial or national commission for specialized examinations, hormone replacement therapy with testosterone, intravenous drug use or penile prosthesis, as the case may be.
