ABSTRACT
The correction of dentofacial deformities through orthognathic surgery is currently considered as one of the pillars of maxillofacial surgery worldwide. There are impressive advances in this area (design of osteotomies, fixation, postoperative stability), however there are complex dentofacial deformities that require descent and advancement movements, orthognathic surgery in patients with cleft lip and palate sequelae or simply the use of segmental osteotomies. The use of regional autogenous grafts in orthognatic surgery becomes an indispensable tool since it facilitates the proper osseointegration of the osteotomized segments since it generates an intimate bone contact and grants the three indispensable principles of a graft, osteogenesis, osteoconduction and osteoinduction. According to our experience and to the literature, the cortical graft of mandibular ascending branch meets the indispensable requirements and takes great advantage in comparison against allografts, xenografts and incuso against chin autografts or maxillary tuberosity.
