Tomographic analysis of fracture lines in mandibular sagittal osteotomies

ABSTRACT
Purpose: A case series study is presented with the objective of evaluate the fracture lines after mandibular sagittal ramus split osteotomy with Hunsuck modification, chisel, mallet and Smith spreaders.
Material and methods: 21 patients (42 mandibular ramus), 14 women (67%), 7 men (33%), between 17 and 53 years with a media of 27.2 years, undertaken orthognatic surgery with mandibular sagittal split ramus at the National Medical Center «20 de Noviembre» ISSSTE in México City in a period of time between 2014 and 2015. Plooij scale was utilized to categorize the fracture pattern on the lingual side of the mandibular ramus with cone beam CT, taken before three weeks post surgical.
Results: A pattern of true Hunsuck SS1 was archived in 60% (25), SS2 29% (12), SS3 2% (1), SS4 9% (4).
Conclusion: The Hunsuck modification with use of chisel, mallet and Smith spreaders produce in the most of the cases (60%) a desired lingual fracture pattern as seen in this study.
