Comparative study of open and laparoscopic approaches for complicated acute appendicitis in pediatric patients

ABSTRACT

Introduction: Acute appendicitis represents the most common surgical pathology in childhood. The objective of this work was to compare post-surgical complications with open and laparoscopic approaches in our hospital in those patients with complicated acute appendicitis. Material and methods: This is a retrospective, observational, descriptive and transversal study, analyzing patient files with the diagnosis of complicated acute appendicitis in a period of three and a half years. Two groups were integrated, 1: open appendectomy and 2: laparoscopic appendectomy. The analyzed variables were: age, sex, preoperative diagnosis, surgical time, days of hospital stay, and early postoperative complications. Results: 91 patients were included who underwent an emergency appendectomy with a diagnosis of complicated acute appendicitis or acute abdomen secondary to appendicitis. Regarding postoperative complications, there was no significant statistical difference from group 1, open approach, with a complication rate of 25.7% (18 patients) to group 2, laparoscopic approach, with 19.04% (4 patients) [p = 0.426]. Hospital stay was longer in patients with an open approach (average of 5.1 days) than laparoscopic patients (3.8 days) with a p = 0.020, statistically significant. Conclusions: In our study, there is no statistically significant difference between these approaches, in terms of postoperative complications when comparing groups of complicated acute appendicitis, except for a reduction in the length of hospital stay in patients managed with a laparoscopic approach.
