Intrahospital weight gain in preterms with normal and low birth weight

ABSTRACT

Objective: To compare weight gain velocity (WGV) and its associated factors in preterm newborns (28 to 34 WGA), according to their birth-weight for gestational age, during their stay at the NICU. Patients: two cohorts; on with adecuate birth-weight (PA, n = 64) and other with low birth-weight (BP, n = 21). Measurements: weekly WGV (g/d) adjusted by: sex, weeks of gestation, days of fasting, days with parenteral nutrition, use of amines, ventilatory support and presence of sepsis. Statistical analysis: t Student, χ2 test and multiple linear regression analysis. Results: During the first week, both groups reported weight loss (WGV PA -2.9 versus BP -1.5 g/d, p = 0.21). From the second to seventh week, WGV were in average › 20 g/d (p = ns). At the eight week only PA group reduced their growth velocity (13.8 versus 30.7, p = 0.03). At discharge, 17.6% (15/85) was in low weight for their corrected gestational age, mostly the BP group (RR = 4.57, IC 95%, 1.8 to 11.3, p = 0.001). Parenteral nutrition had a positive influence in WGV, contrary with the ventilatory support. Conclusion: In all preterm infants the rate of intrahospital growth is negative the first week and reverts in the second one. From month one, the BP accelerates more its growth.
