ABSTRACT

Introduction: Congenital diaphragmatic hernia is an anatomical defect of the diaphragm of unknown etiology; it is related to important morbimortality and severe respiratory symptomatology of early onset in the neonatal period. Factors of poor prognosis are a low APGAR at the first minute, low birth weight, early presentation of the symptomatology and involvement of the liver within the defect. Presentation of the case: preterm male neonate of 36.2 weeks of gestation with neonatal depression and severe respiratory distress beginning at 12 hours after birth as a manifestation of a right congenital diaphragmatic hernia. The patient developed pulmonary arterial hypertension. Surgery was performed at 96 hours of life with satisfactory evolution. Conclusion: With multidisciplinary management the patient survived, despite having poor prognostic factors.
