Gestational choriocarcinoma after term pregnancy. A case report

ABSTRACT

Background: Choriocarcinoma is a tumor of the trophoblast with high capacity of angioinvasion and dissemination, the organs most frequently affected are: lung, vagina, central nervous system, gastrointestinal system, liver and kidney. The incidence of gynecological choriocarcinoma depends to the geographical area analyzed: in Europe and the United States of America, one case is reported in 30,000 to 40,000 pregnancies, in Asia one in 500 to 3,000, while in Mexico one case is reported In 10,000 pregnancies. The Tabasco state doesn't have incidence statistics. In the hospital Dr Gustavo A Rovirosa Perez this is the first case reported in a term pregnancy.
Clinic case: A 27-year-old patient diagnosed of postpartum choriocarcinoma, the suggestive clinical data were abundant and persistent transvaginal bleeding in the puerperium associated with high concentrations of B-Hormone Human Chorionic Gonadotropin (B-HGC) and ultrasonographic image. The diagnosis was confirmed by cavity biopsy. An EMA-CO chemotherapy protocol was initiated, which could not be terminated by patient death.
Conclusion: Choricarcinoma is a tumor of low incidence but with a mortality rate of 10 to 15%, so its detection in the early stages is of utmost importance. If there are elements of clinical suspicion, the necessary studies must be performed to reach the diagnosis in the shortest possible time, because if it is detected in a opportunitly time, the patient can reach a survival rate of 85%.
