ABSTRACT

Introduction: Coronary artery fistulas are characterized by an abnormal communication between the coronary artery and a cardiac chamber or towards the coronary sinus.
Case presentation: a 28 years old male patient, 10 days before admission showed a pain who woke him up when sleeping, oppressive pain sensation, very intense, with a retrosternal location, associated to dyspnea.
Material and method: Angiography procedure was made.
Discussion: Coronary fistulas etiology may be congenital-origin or produced by complications of an invasive procedure such as septum myomectomy for hypertrophy myocardiopathy treatment. This case corresponds to a variant kind of Type II, with communication to the venous sinus and towards the right ventricle.
Conclusions: Coronary fistulas are infrequent abnormalities; they may affect any age group.
