Cadaveric donor block renal transplant with expanded criteria. Case Report

ABSTRACT
The National Registry of Transplants reported in 2016 a total of 852 deceased donor kidney transplants from a waiting list of 12, 477 patients. Due to the organ shortage, the criteria for acceptance of decesed donors have been extended over the years, which is why the criteria of expanded donors are born.
Case report: A 56-year-old woman with brain death secondary to aneurysm rupture, classified as a donor with expanded criteria for meeting the following criteria; age greater than 55 years, cerebrovascular disease and systemic arterial hypertension, in bench surgery, presence of extensive plaque in the aorta and decreased kidneys. Male recipient of 43 years of age, with renal function post-transplant at 48 hrs with diuresis of 4 liters in 24 hours and baseline creatinine decrease from 6.1 mg/dL to 4.6 mg/dL. High on the sixth day post-transplant with creatinine of 1.1 mg/dL, with a GFR of 56 ml/min at one month and 79 ml/min at 8 months with FG.
Conclusion: En bloc transplant of adult donor to adult recipient in an excellent option in the presence of multiple renal vessels and / or extensive atherosclerotic disease and a means that ensures a greater number of functional and viable nephrons.
