Portal vein thrombosis after endoscopic and bariatric surgery, literature revision

ABSTRACT
Introduction: Portal vein thrombosis is a rare but potentially lethal complication after laparoscopic procedures. Obese patients that go under bariatric surgery have an increased risk of developing venous thromboembolism due to their hypercoagulable and inflammatory state. Bariatric procedures are not free of complications; it is reported that its incidence is as high as 7%.
Material and methods: An electronic search was carried out on the PubMed, Medline, Central, Embase, Ebscohost, and Cochrane Central platforms for the database of articles published from January 2000 to December 2017. The following key words were used: Portal venous thrombosis, laparoscopic surgery, Roux-en-Y gastric bypass, treatment of complications, treatment and diagnosis. An analysis of 45 articles in this review was made emphasizing the evaluation, diagnosis, and treatment of portal venous thrombosis secondary to laparoscopic bariatric surgery.
Conclusions: The diagnosis and treatment of portal vein thrombosis secondary to laparoscopic surgery to prevent intestinal ischemia and possible necrosis should not be delayed. Due to its wide range of symptoms, a high index of suspicion is required. Anticoagulant therapy is generally satisfactory to prevent thrombus extension, achieving partial recanalization and avoiding new thrombotic events.
