ABSTRACT

Introduction: so ated sp en c cysts are rare n ch dren potent a y fe threaten ng and often d fficu t to d agnose. We ntroduce a comp cated case w th bacter a super nfect on and abscess ng to fac tate recogn t on and ear y d agnost cs and d scuss about best treatment opt ons.
Case Report: 5 year o d boy brought to the R by h gh fever abdom na pa n and vom t ng. n the b ood test showed eukocytos s w th e evated acute phase reactants negat ve b ood cu ture and pos t ve stoo cu ture for Salmonella serogroup D. n the chest rad ograph was obse ved a eft basa nfi trated w th p eura effus on and n the abdom na u trasound a arge cyst c ntrasp en c es on. W th the d agnos s of g ant sp en c abscess ntravenous ant b ot c treatment and repeated percutaneous dra nage were estab shed unt the defin t ve surgery w th cyst c part a remova and capsu e fenestrat on. n the cu ture of the abscess t grew a Salmonella of the same serogroup. s topatho og ca exam nat on of the spec men was cons stent w th pr mary ep the a cyst.
Conclusions: Sp en c cysts shou d be cons dered n the d fferent a d agnos s of any pat ent w th s gns suggest ve of patho ogy n the eft upper quadrant. n recent years there has been great progress n ts hand ng that n the case of ch dren t s usua y conservat ve.
