ABSTRACT

Deep vein thrombosis (DVT) associated with extrinsic compression due to the presence of a perivascular endometrioma is a rare clinical scenario. In the present article, we report the case of a 41 year-old female patient with diagnosis of an endometrioma located in the right pelvis, which provoked moderate edema and refractory pain in the pelvis and her right lower extremity, exacerbated during her menstrual period. Imaging confirmed that the endometrioma caused extrinsic obstruction of the right common iliac vein resulting in limitation in the venous return. Systemic anticoagulation was initiated, however the symptoms persisted. The patient underwent surgical resection of the endometrioma allowing restoration of the deep venous return and with resolution of the symptoms.
