ABSTRACT

Introduction. Infrapopliteal aneurysms are extremely rare. Only 34 incidents have been reported in the literature, and only 6 of them involved the anterior tibial artery.
Clinical case. This is a 68-year-old female patient who went to the outpatient service of Trauma and Orthopaedics Department, referring a 6-month clinical course with the presence of a tumor and occasional pain on the left foot. She was admitted at the Hospital General de Ciudad Juárez, in charge of the aforementioned department, scheduled for resection of a presumable synovial cyst on the left foot. In the operating room, a pulsatile tumor was found and the diagnosis of aneurysm of the anterior tibial artery was made. It was ligated and resected. The surgical specimen was sent to pathology and an aneurysm with fragments of arterial wall with ectasia and grade IIa atherosclerosis were reported.
Conclusion. The aneurysms of the anterior tibial artery are exceptional. The etiology it is undetermined. It is important to diagnose this identity and according to the patient status, individualize the treatment.
