Airway pressure release ventilation in a patient with thorax´s trauma

ABSTRACT

Introduction: In the deaths produced by trauma, between 15 and 25% of those are produced by thoracic trauma. The flail chest appears in 10 to 15% of them, usually accompanied by pulmonary contusion, and is attributed to it from 15 to 20% of the deaths. Mechanical ventilation is one of the pillars of treatment in these cases. Nevertheless, there are few publications that evaluate the potential benefits of the ventilatory modalities used. Objective: To describe the behavior and evolution of a patient with flail chest and pulmonary contusion who suffered a severe respiratory distress. Clinical case: Patient who suffered a severe thoracic trauma, and required mechanical ventilation and thoracoplasty during its evolution. That is why he was ventilated with airway pressure release ventilation. Conclusions: Airway pressure release ventilation is proposed as a strategy that facilitates the stability of the thoracic wall and alveolar recruitment which theoretically offers advantages in pulmonary protection and in the systemic inflammatory response.
