Treatment by minimally invasive surgery of uretero-vesical iatrogenias

ABSTRACT

Introduction: Iatrogenic uretero-vesical injuries are infrequent, but they have a great impact on the integrity of the patients who suffer from it. Objectives: To describe the results of the implementation of minimally invasive surgery in the solution of iatrogenic injuries of the urinary tract. Methods: A retrospective, descriptive, cross-sectional study was conducted in 54 patients at the National Center for Minimum Access Surgery from 2006 to 2015. Endourological, percutaneous and laparoscopic procedures were performed to treat the injuries. Clinical, epidemiological variables of the patients in the series and perioperative results were included. Descriptive statistical methods were used. Results: 54 patients were included, most of them female (98.2%), with an average age of 45.4 years. The most injured organ was the bladder (59.6%) in its retrotrigonal portion. The most frequent injuries were fistulas (57.1% of vesicovaginal and 19.6% of ureteral-vaginal), the majority related to a history of hysterectomy by open surgery (76.3%). The injuries were treated with a single minimally invasive surgery in 61.1% of the patients and in the rest different techniques were combined. The feasibility of the procedures was of 100%. The success in the solution of the injury was 94.5%. There were no complications in 89.1% of the patients. Conclusions: Minimally invasive surgery is a feasible therapeutic option, which is also safe and with a high percentage of success for iatrogenic uretero-vesical injuries.
