Growth of two malignant tumors of different histologic type on a sebaceous nevus of Jadassohn

ABSTRACT

The nevus sebaceous of Jadassohn (NS) is a complex, congenital and benign hamartoma. It represents a fertile field for the development of secondary adnexal neoplasms, occasionally malignant, being the most frequent the basal cell carcinoma (BCC). Sebaceous carcinoma (SC) is an aggressive malignant tumor, very uncommon on a NS. We report the case of a 36-year-old female with an alopecia patch from birth, with accelerated and symptomatic growth in the last four months. The physical examination revealed a red-orange alopecia plate, triangular in shape, with a maximum diameter of eight centimeters, verrucous surface, associated with a red-bluish papule, in the left parietal region. On suspicion of NS associated with CBC, surgical removal was decided. Histopathology concluded NS with pigmented nodular BCC and well-differentiated SC. The simultaneous occurrence of two carcinomas in a NS is rare, and occasionally, the SC corresponds to one of them. The SC was seen on a NS without metastasis or cutaneous recurrence despite the size and its late extirpation, which confirms a non-aggressive behavior and a favorable prognosis.
