Acetaminophen overdose: diagnosis and approach in adults

ABSTRACT

Acetaminophen overdose is the main cause of acute liver failure. It can happen because of an acute intake or an intoxication with stepped doses. The most study mechanism of injure is the accumulation of N-acetyl-parabenzoquinoneimine that deceases the reserves of the antioxidant glutathione causing oxidative stress in the mitochondria. The clinical presentation is divided into four phases ranging from an unspecified presentation in the first 24 hours to the peak of liver dysfunction at 72-96 hours. The n-acetylcysteine is the recommended antidote. Hepatic insufficiency is a rare complication, but is the main cause of mortality.
