ABSTRACT

The aesthetic restoration of the anatomy and function of the eyelid is the surgical goal in the reconstruction of the traumatic defects of the lower eyelid. The objective is to combine the use of advancement flaps with the use of chondral grafts for the reconstruction of extensive post-traumatic lower eyelid defects. A case is presented of a man in the second decade of his life was thrown against asphalt in a motor vehicle accident. This caused a defect by traumatic avulsion of the lower eyelid that involved 60% of the total thickness, compromising the tarsus and conjunctiva. The eyelid was reconstructed by means of a type Mustardé rotation and advancement flap, and atrial cartilage graft placement. According to the reconstructive principles described by Mustardé, we have different techniques for the reconstruction of extensive palpebral defects, which cause loss of the structural support of the eyelid, compromising the function of the eyeball. In our case, once the rotation flap of the cheek was made, we opted for the reconstruction of the vertical defect by means of an auricular graft of the shell, since it is a versatile cartilage that, upon contact with the bulbar and palpebral conjunctiva, favors an adequate means of support.
