ABSTRACT

Leptospirosis is an acute infection. Respiratory involvement is frequent, but it is usually mild and undiagnosed, it is unusual at the onset of the disease. In recent years, a change in clinical presentation has been reported. There is higher incidence of lung involvement. This article proposes to describe a clinical case of leptospirosis and acute respiratory failure, treated at Dr. Miguel Enriquez Teaching Clinical - Surgical Hospital. This the case of a 51-year-old patient with epidemiological history. Clinically, he presented acute febrile symptoms, manifestations of Weil syndrome associated with lung injury with manifestations of pulmonary hemorrhage and rapid progression to acute respiratory failure. Serological tests confirmed the diagnosis of leptospirosis. Supportive respiratory treatment was performed with non-invasive mechanical ventilation, with excellent clinical and radiological evolution.
