Superficial temporal artery aneurysm: ¿what to do?; a case report

ABSTRACT

Superficial temporal artery aneurysms represent an infrequent pathology. They may appear secondary to trauma (pseudoaneurysms) or they may be primary (true aneurysms). Due to the lower incidence of this pathology there is no consensus on the best treatment modality. Surgical resection tends to be the most frequently used therapeutic option because of its high effectiveness and low risk for complications. The objective is to describe the types of superficial temporal artery aneurysms and analyze the existing therapeutic options to improve decision making in this pathology. The use of intraoperative Doppler ultrasonography for the resection of the lesion is also reported. Clinical case: The authors present the case of a male patient, 45 years old, who presented with a progressive right temporal region lump, which caused ipsilateral hemicranial headache and perilesional paraesthesia. There was no history of trauma to the anatomical region. The presence of a superficial temporal artery aneurysm was corroborated by angioCT, so the authors decided to perform afferent and efferent artery ligation guided by Doppler ultrasonography followed by resection of the lesion, achieving complete resolution of the preoperative symptoms with a satisfactory aesthetic result. The use of intraoperative Doppler ultrasonography for early detection of the aferent and eferent artery in this case proved to be an effective tool in order to achieve a proper and safe aneurysm resection.
