Malignant fibrous histiocytoma of the spermatic cord

ABSTRACT

Background: Malignant fibrous histiocytoma is an infrequent tumor that represents 0.2-0.6% of paratesticular tumors. Objective: To present the case of a paratesticular malignant fibrous histiocytoma, the first diagnosed in our province in 54 years. Clinical Case: 52-year-old male patient, driver of heavy equipment since his youth, for which he suffered multiple scrotal injuries. Thirty days before his admission he noticed an increase of the right testicle with pain that required anti-inflammatory treatment. Upon physical examination, a tumor of approximately 6 cm, hard, irregular surface with multiple nodulations, painful that did not transilluminate, was found above the right testicle. Laboratory tests were normal, including alpha-fetoprotein and β-HGc, with the erythrocyte sedimentation rate and LDH being abnormal. Testicular ultrasound showed a mass of approximately 6 cm, above the right testicle without involvement of the same. The right inguinal exploration was determined, identifying a tumor dependent on the spermatic cord without infiltration to the testicle. Total right orchiectomy was performed and the histopathological study revealed a malignant fibrous histiocytoma. Conclusions: Malignant fibrous histiocytoma is an extremely rare neoplasm within paratesticular tumors. The only therapy with curative possibilities is surgery. The use of chemotherapy and radiotherapy are usually not effective.
