Handling of the adipose ball of Bichat. Contribution to the conventional bichectomy technique

ABSTRACT

The BFP was first described by Heister (1732), who believed that this structure was glandular in nature so he called it «glandula molaris». It is attributed to Bichat the recognition of the true nature of the Bichat adipose pad (BAB) therefore it is commonly known as the boule de Bichat or bolle graisseusse in French; wangenfettpfropf or wangenfettpolster (wangen = cheek + fett = fat + polster = bag) in German; masticatory fat pad or buccal fat pad (BFP) in English. In the present work a scientific revision of the surgical removal of the Bichat adipose pad is carried out; as well as a contribution to the conventional intraoral technique used for the removal of the same, whose main objective is to improve the contour and facial profile by framing the face and highlighting the bony characteristics of the malar region, decreasing the volume of the middle third in the patients that require the removal of the adipose pad or that they present themselves to the facial surgery consultation which present volume in the region of the cheek possibly due to a displacement of the fat pad (pseudoherniation).
