ABSTRACT

Introduction: the importance of bipolar disorder has grown due to a broader clinical concept of the disease, the bipolar spectrum; hence its prevalence in the population. Its chronic course, early onset and misdiagnoses favor a high level of disability. Objective: to determine the evolutionary characteristics that distinguish bipolar disorders from adulthood to the elderly in patients treated at the Dr. Luis San Juan Pérez Provincial University Psychiatric Hospital in Villa Clara during 2017. Methods: a descriptive investigation was conducted. A sample with two groups was selected: 60 adults (between 19 and 59 years old) and 60 elderly patients to whom a structured form with biopsychosocial variables and psychological and statistical methods was applied. Results: towards the third age the disorder adopts peculiar characteristics. The depressive phase of the disease predominates, the association with other organic disorders and the duration of the crises exceed the month, the cycle or the change of pole decreases, the frequency of admissions to hospital decreases and therapeutic adherence improves, polypharmacotherapy becomes very significant, there is a tendency to adapt to social functioning and complications decrease; however, suicidal behavior increases and deterioration rates are more significant. Conclusions: characteristics were identified that could distinguish the peculiarities of the bipolar disorder in the elderly, aspects that help individualize the protocol of preventive, diagnostic, therapeutic actions as well as the psychosocial intervention in this condition.
