Relevance of articulator mounting as an auxiliary method for differential diagnosis between class III and pseudo-class III malocclusions

ABSTRACT

Background: Semi-adjustable articulators allow having a reproducible view of the upper and lower jaws outside the mouth and provide knowledge of their behavior, such as the discrepancies between centric relation and habitual occlusion. Objective: We aimed to prove the functionality of the articulator as a diagnostic tool in the orthodontic treatment of growing patients. Material and methods: We treated a 10-year-old patient with anterior crossbite and initial diagnosis of class III malocclusion. Measures were recorded for mounting on the articulator. A forward positioning of the mandible was evident due to a postural reflex acquired by existing premature contact points, giving the appearance of a true class III malocclusion. The treatment consisted of use of protraction facemask in combination with a bonded bite block-type appliance with hyrax palatal expander for nine months to achieve correction of the malocclusion. Results: Premature contact points previewed in the articulator were eliminated and corroborated in the mouth, which allowed for an adequate intermaxillary relationship with the condyles positioned in centric relation and a coinciding centric occlusion. The crossbite was corrected, restoring functionality of the stomatognathic system and thus achieving an improvement in the patient’s profile. Conclusions: Articulator mounting is an effective method to reveal undetected malocclusions as is the case of the pseudo-class III caused by acquired habitual reflex.
