ABSTRACT

We report a case of a 76-year-old male with a proliferating trichilemmal cyst. It is a benign tumor originated in the external root sheath in the hair follicle. It exhibits predilection for the scalp due to the higher density of hair follicles. These lesions are approximately five times more common in women, and the mean age is 65 years old. The typical proliferating hair cyst or proliferating trichilemmal tumor is a large (up to 25 cm) exophytic neoplasm confined almost exclusively to the scalp. The behavior is generally benign, locally destructive behavior and metastases to regional lymph nodes are rare. Histopathological findings are widespread trichilemmal keratinization. Treatment of choice is complete surgical excision.
