Esophagus leiomyoma and its endoscopic transhiatal resection.A case report

ABSTRACT

Objective: To communicate the experience from the first case of laparoscopic resection of an esophageal leiomyoma from the bronchodiafragmatic segment carried out in our hospital.
Setting: Third level health care regional Hospital.
Description of the case: On November 2003, a transhiatal laparoscopic resection of an esophageal leiomyoma of the third lower part was practiced to a 42-year old male patient, who was incidentally diagnosed this tumor during the tomographic imagining practiced due to a biliary pancreatitis that the patient had presented four months before. The pre-operative study included SEGD, esophagus endoscopy with a sample for biopsy and axial computed tomography. The enucleation of the 30 cm-length and 70 grams of tumor weight concluded successfully by the means of a transhiatal laparoscopic boarding, with the verification of the esophageal mucosa integrity through trans-operative endoscopy. The patient was discharged 30 hrs after the surgery, tolerating liquids and presenting a normal thorax x-ray. A control esophageogram was practiced 30 days after the postoperative period without evidence of reflux, remaining asymptomatic and without any radiological changes after 180 daysregarding the first study. There was no malignity evidence in the histopathologic study and the diagnosis was confirmed through studies of immunohistochemistry.
Conclusion: Leiomyoma is a benign tumor that rarely occurs in the esophagus and even more rarely presents the size reported here. This report exposes the possibility of a new approach and its benefits.
