ABSTRACT

Background: The RS3PE syndrome (remitting seronegative symmetrical synovitis with pitting edema), or McCarty syndrome, is an infrequent rheumatologic clinical picture, which mainly affects people over 70 years old; it manifests acutely as polyarthritis and symmetric synovitis besides edema that leaves fovea especially in hands.
Clinical case: A 78-year-old male patient with acute clinical onset, paresthesia and localized edema in hands, associated with synovitis and manifested with pain and limit for functionality, fulfilling all the diagnoses for RS3PE syndrome, evolved with excellent response to corticosteroids and its origin was not evidenced.
Conclusions: Although RS3PE syndrome has benign prognosis, it has been related to neoplasms, so, these must be discarded when establishing the diagnosis.
