Wünderlich syndrome

ABSTRACT

Background: Spontaneous subcapsular or perirenal renal hemorrhage of nontraumatic etiology is a rare condition. The origin of this entity is varied, more than 50% corresponds to a tumor disease, adenocarcinoma is the most frequent cause, followed by angiomyolipoma.
Clinic case: A 51-year-old male patients with a history of type 2 diabetes who began his clinical picture with stabbing pain in the right renal fossa of progressive intensity, irradiated to the right hypochondrium, accompanied by nausea, vomiting of food content on several occasions, general problems and progressive dyspnea. Upon physical examination with pain in the renal fossa and right hypochondrium without data of peritoneal irritation. A simple and contrasted CT reported a mixed lesion of 13 x 10 cm on the upper right pole, so it was decided to perform the retroperitoneal examination with radical nephrectomy, the sample was sent to pathological study.
Conclusions: Wünderlich syndrome or spontaneous renal hemorrhage may be due to varied causes; neoplastic origin is the most frequent; it is a rare disease in our environment and in Mexico there are few cases reported.
