Autoimmune-chronic active hepatitis in children. Experience in 20 cases.

ABSTRACT

Objective. To know the continuous evolution in 20 children with autoimmune-chronic active hepatitis (A-CAH) treated at the Hospital Infantil del Estado de Sonora.
Material and methods. A restrospective revision wide included the following variables, age, gender, personnal history, signs and symptoms, laboratory tests, hystologic pattern of tissue obtained by biopsy, treatment, evolution and mortality.
Results. They were eleven males and nine females; predominant signs were hiporexia, fatigue, hepatomegalia and ictericia. Five patients presented moderate anemia, leucopenia and thrombocitopenia; 16 presented hypergammaglobulinemia, all went true percutaneous hepatic biopsy, histological pattern was: rupture in the border plate, focalnecrosis colangiolar proliferation, fibrotic bridges, four showed precirrhotic changes, they all received prednisone for 3 and 9 year periods and in four cases azathioprine was combined; six cases presented clinical, biochemical and histological remission and six with biochemical remission but presented inflammatory activity, and 3 died of cirrosis and hepatic insufficiency.
Conclusion. Until now the A-CAH, etiology in unknown and treatment has not changed in the last 50 years which in prednisone alove or combined with aziathioprine.
