Miliary tuberculosis in a patient with abortion after in vitro fertilization

ABSTRACT

Background: Tuberculosis is a worldwide health care problem. The World Health Organization estimates that about one third of the world population is infected. Miliary tuberculosis is the result of a lympho-haematogeneous dissemination of Mycobacterium tuberculosis. Genital tuberculosis is one of the most common causes of primary infertility in regions where exists a high tuberculosis prevalence. It is almost always secondary to the presence of tuberculosis in another primary infection site, usually lung. Due to the paucibacillary nature of genital tuberculosis, demonstration of Mycobacterium tuberculosis in genital tract is not always possible. The diagnosis of a primary focus of tuberculosis through molecular tests (Xpert MTB/RIF) is enough to start treatment and suspect miliary tuberculosis as the cause of primary infertility.
Clinic case: A 29-year-old female patient with in vitro fertilization who developed transfusion associated circulatory overload syndrome after an inevitable miscarriage with miliary tuberculosis as the cause of primary infertility.
Conclusions: Tuberculosis is a disease with very versatile clinical manifestation. In endemic zones of tuberculosis, in patients with risk factors and respiratory symptoms, it always must be a differential diagnosis.
