Historical development of the Transplant Service at the General Hospital of the National Medical Center “La Raza”, IMSS

ABSTRACT

Objective: To describe the development of a multiple organ transplant program.
Setting: Third level health care hospital.
Results: We reviewed the records of the Transplant Service of the General Hospital National Medical Center “La Raza” (IMSS) based on the monthly, quarterly, and annual reports to the General Direction of the Hospital and the National Transplants Registry (NTR), as well as the surgical records of each patient and the morbidity and mortality committee records, during the period of January 1989 to December 2000. During these 12 years, the renal transplant program started first with living donors and evolved to cadaveric donors. This last type of donation evolved from a regional level to a national one among the population covered by the Social Security Institute (IMSS), through the implementation of the “Pro-donation” program. Along this time, the technology for the development of the transplant program evolved with the use of the pulsatile hypothermic perfusion pump (Mox-1100, Watters, Inc.), which allows to preserve renal allografts for a longer time, using the machine as a prognostic index of performance. On the other side, it allowed us to develop other programs, such as the hepatic transplant, since the kidneys can be preserved longer while transplanting the liver and heart. In parallel, the cardiac orthotopic and heterotopic tranplant program developed, uni- and bi-ventricular support systems were implemented as a bridge for cardiac and the first heart-lung transplants.
Conclusion: The historical development of the Service has evolved from a basically renal transplant program to a multiple organ transplant program, concentrated in a multidisciplinary unit that has responded to the demand of end-stage patients at a third level health care institution.
