ABSTRACT

Introduction: Vogt-Koyanagi-Harada disease is a rare granulomatous inflammation that affects pigmented structures. A miotic pupil that does not respond to mydriatic medication represents an event with a risk of complications in cataract surgery.
Objective: to present the results obtained with the intraoperative application of iris retractors in a cataract operation in a patient with Vogt-Koyanagi-Harada disease.
Case presentation: 50-year-old female patient with Vogt-Koyanagi-Harada syndrome and total cataract in the left eye. The patient was compensated for her inflammatory and hypertensive ocular symptoms. Her prior informed consent was requested. Iris retractors were used in the operation, and extracapsular extraction with insertion of intraocular lens was performed, without complications and without adverse events.
Conclusions: the iris retractors were useful to achieve good surgical results in the patient. A best-corrected visual acuity of 0,2 was obtained.
