Dental health services utilization by children below 5 years age with social security

ABSTRACT

Objective. To determine the factors modifying dental health services utilization in children 4 years old and younger in a third-party dental insurance system.
Material and methods. A cross-sectional study was carried out in day care centers of the Instituto Mexicano del Seguro Social in Campeche, Mexico. A total of 189 children were selected; the sociodemographic and socioeconomic data were obtained using a questionnaire for the mothers. Only 155 children were included in the final analysis, after meeting inclusion and exclusion criteria. The use of dental services was measured as any dental visit in the 12 months prior to the interview. Data were analyzed in a multivariate model in STATA 8®.
Results. The overall use of dental health services rate was 31.0%. Results adjusted by multiple logistic regression showed that use of dental health services was significantly related to oral health needs (OR = 6.40; CI 95% = 2.04 - 20.06), female sex (OR = 2.61; CI 95% = 1.21 - 5.67), and higher level of parental schooling (OR = 2.40; CI 95% = 0.99 - 5.83). We observed a negative relationship between use of dental health services and sequence of birth order within the family (OR = 0.42; CI 95% = 0.20 - 0.92).
Conclusions. The use of dental health services in this toddler population was mainly associated with oral health needs. Others variables such as parents’ schooling and birth order, and sociodemographic features such as sex, contributed to explain the use of dental health services. These findings have implications for the design of dental care delivery systems for children.
