The healing process of wounds is incredibly complex as it depends on an intricate interaction between the body’s cells to restore the barrier function of the skin. This process can be affected by several concomitant factors, such as diabetes, malnutrition and immunosuppression, resulting in a chronic wound. Historically, wounds in the oral and maxillofacial region have been difficult to manage due to multiple factors, either due to the irregular contours of the face as well as the aesthetic and functional components, and the management of these wounds is complex and prolonged. In the last 10 years, vacuum assisted closure has been implemented in General Surgery to close large and difficult wounds in the body and recently the effectiveness and easy handling of wounds with vacuum assisted closure has allowed its application in the maxillofacial region. We present the case of a male patient with a diagnosis of mandibular osteomyelitis, resection of the bony segment and reconstruction plaque placement as the first time, taking and application of microvascularized graft of fibula in the second surgical time and dehiscence and infection of the surgical approach as well as the application and use of the vacuum assisted closure system (VAC) as a feasible and non-invasive method for its treatment.
