ABSTRACT

Introduction: The ischemic heart disease is an important problem in productive adults. Early intervention in this group of patients had a direct impact on the outcome.
Objective: To report the morbidity and mortality of patients with acute coronary syndrome, who were transported to another hospital.
Design: Retrospective study.
Setting: ICU of a tertiary care hospital, Guadalajara, Jalisco; Mexico.
Patients: Eight hundred and ten patients with acute coronary syndrome transported by a surface mobile unit from January 2001 to June 2005.
Results: Acute myocardial infarction was present in 563 (76%) of patients and transcutaneous pacemaker and thrombolysis were used in the emergency room before transportation in 60 (10.6%) and 52 (9.4%), respectively of these. Forty three (53%) patients developed cardiogenic shock; 68 (12%) required assisted mechanical ventilation. Vasopressors were used in 131 (23.2%) of patients with myocardial infarction and in five (2%) with unstable angina. Inferior myocardial infarction was the most common presentation. Four patients died and death was associated with lethal arrhythmias and cardiogenic shock in two of these.
Conclusions: Acute myocardial infarction is the most common disease whose requires interhospital transport in our population; the most important risk factor of death may be related with lethal arrhythmias and cardiogenic shock.
