ABSTRACT

Objective: To assess the viability and results of a technique without pneumoperitoneum in laparoscopic surgery.
Setting: General Hospital.
Design: Series of cases, without control group and without statistical analysis.
Patients and methods: One hundred-eighty-nine patients were subjected to laparoscopy without pneumoperitoneum, using a parietal traction system. The following variables were assessed: age, gender, type of surgery performed, surgical time, post-operative pain intensity, hospital stay, conversion to laparoscopy with pneumoperitoneum, and complications.
Results: One hundred-thirty seven women and fifty-two men were operated; average age was 43 years, ranging from 12 to 82 years. We performed 150 cholecystectomies, 23 appendicectomies, and 16 gynecological surgeries. Mean surgical time was of 59 minutes, ranging from 20 to 130 min. Pain was assessed by means of an analog visual scale, from 0 to 10, 155 (82%) patients referred pain in the 0-5 range. Three conversions to open surgery had to be made. Ninety-seven percent of the patients was released from the hospital at 24 h. Morbidity was of 4.2%, five patients with infection of the umbilical wound and three with a periumbilical hematoma.
Conclusion: The used technique is a viable procedure and offers good results.
