ABSTRACT

Purpose: To determine the basic nosological elements that let us know precisely that esotropia is from congenital origin, independently of the age when it occurs or is diagnosed and how those elements interact to understand better the genesis of the congenital esotropia (CE).
Material and methods: A prospective, longitudinal, observational and descriptive study of patients with (CE) was realized. All patients underwent a complete strabological history and a sensorial, refractive and motor examination with: horizontal and vertical versions, detection of hyper and hypo functions, dissociated movements, type of nystagmus, torticolis, visual capacity, refraction, fusion proofs and detection and measurement of the grade of amblyopia. Furthermore, we realized a video of the slow movement of the horizontal version, as well as mesopic and escotopic conditions through infrared ray. An electroculography (EOG) was also realized.
Results: 65 patients with CE where analyzed. The four most significant clinical elements to determine the congenital origin of esotropia were: Limitation of the abduction (LABD) in 95.4%, horizontal incommitance (IH) in 67.7%, dissociated vertical deviation (DVD) in 66.15% and latent nystagmus (NL) in 60%.The coefficient of intensity of relation calculated by Q of Yule was 0.5342.
Conclusions: We demonstrate the diagnostic evidence of CE from the correlation of four basic clinical elements that determine the congenital stigma, independently of the age of the patient.

Key words: Congenital esotropia, horizontal incommitance, nystagmus, electroculography, video film, congenital nystagmus.
