Linfoma orbitario de células del manto atendido con quimioterapia

RESUMEN

Introducción: Los linfomas son tumores sólidos de células linfoides y se subdividen en Linfoma Hodgkin y linfoma no Hodgkin (LNH). El caso a tratar es un tipo de LNH ubicado en la órbita ocular de una paciente. Su incidencia en la población aumenta con la edad, pudiendo presentarse incluso por encima de los 80 años.
Presentación del caso: Se presenta una paciente de 73 años de edad con exoftalmos bilateral y se le realiza un examen clínico ocular y sistémico. Los estudios imagenológicos muestran la tumoración de ambas órbitas, mayor en la derecha. El diagnóstico inicial fue pseudotumor inflamatorio de la órbita, confirmado por una primera biopsia. Debido a la ausencia de mejoría clínica con los esteroides, se repitió la biopsia. En esta ocasión, se tomó la mitad de la tumoración orbitaria izquierda. Según los estudios realizados por la patóloga, se trata de un Linfoma de Células del Manto (LCM), secundario de la órbita, de alto grado de malignidad. La prueba histológica de ciclina fue crucial para el diagnóstico positivo y diferencial.
Resultados: Se decidió el tratamiento con quimioterapia con el esquema terapéutico CHOP, (ciclofosfamida, adriamicina, vincristina y prednisona). Los resultados fueron satisfactorios, con una sobrevida actual de 1 año de evolución.
