Implicaciones en Neuroanestesia durante la pandemia por COVID-19

RESUMEN

El COVID-19, declarado pandemia por la Organización Mundial de la Salud en marzo de 2020 ha generado más de 11,000,000 casos y 500,000 muertes a nivel mundial. El riesgo de infección en procedimientos generadores de aerosoles y estancia en procedimientos prolongados, ha impactado la atención neuroquirúrgica y endovascular generando importantes implicaciones para neuroanestesiólogos y atención perioperatoria en general debido al riesgo de infección de pacientes y personal y la asociación del SARS-CoV-2 con pacientes neuroquirúrgicos.
Objetivo. Presentar una serie de recomendaciones basadas en la evidencia actual sobre las implicaciones para neuroanestesiólogos y el manejo perioperatorio del paciente neurocrítico y neuroquirúrgico durante la pandemia por COVID-19.
Material y métodos. Revisión bibliográfica mediante búsqueda sistemática de palabras clave relacionadas con neuroanestesiología y manejo perioperatorio del paciente neuroquirúrgico y neurocrítico durante la pandemia por COVID-19.
Resultados. El paciente neuroquirúrgico y neurocrítico plantea importantes retos que implica rápida capacidad de respuesta, establecimiento de protocolos de manejo, estratificación multidisciplinaria, disponibilidad de infraestructura, EPP y personal que aseguren su atención.
Conclusiones. El rol del neuroanestesiólogo es fundamental en organización y manejo de la pandemia; se debe mantener seguro, protegido y actualizado en las consideraciones específicas del paciente neurocrítico en esta “nueva normalidad”.
