Tratamiento del linfoma no Hodgkin con rituximab y quimioterapia CHOP. Experiencia de 6 años en el Hospital Regional “General Ignacio Zaragoza” ISSSTE

RESUMEN
El tratamiento del linfoma no Hodgkin ha cambiado en los últimos 20 años. Rituximab es un anticuerpo monoclonal dirigido contra el antígeno CD20; la asociación de éste con quimioterapia convencional CHOP (ciclofosfamida, doxorrubicina, vincristina y prednisona) ha demostrado buena respuesta e incremento en la supervivencia de los pacientes. Se realizó un estudio prospectivo, longitudinal en 60 pacientes durante un periodo de 5 años en el Hospital General Ignacio Zaragoza del ISSSTE para determinar respuesta al tratamiento y supervivencia libre de enfermedad; se estratificaron los pacientes mediante clasificación de Ann Arbor y el Índice Pronóstico Internacional (IPI); se obtuvieron muestras histopatológicas y determinación antigénica del CD20; se administró esquema de rituximab y CHOP en 8 dosis. Se obtuvo una respuesta hematológica con remisión completa de 54 pacientes (90%) con recaída de 6 pacientes (10%), 59 pacientes (98.3%) se mantienen en remisión y sobrevivencia del 100% (60 pacientes) en un periodo de 5 años. El esquema rituximab + CHOP en tratamiento de los pacientes con linfoma no Hodgkin ofrece una alternativa eficaz, adecuada tolerancia, perfil de seguridad óptimo e incremento significativo en el periodo de supervivencia libre de enfermedad.
