Meningitis por Streptococcus pneumoniae serotipo 35b en lactante vacunado. Reporte de caso

RESUMEN
La meningitis bacteriana es una causa importante de morbimortalidad en niños y niñas menores de cinco años.
Streptococcus pneumoniae es el principal agente etiológico; su epidemiología ha cambiado tras la introducción de
las vacunas conjugadas (pcv10 y pcv13). Se ha producido un reemplazo de los serotipos debido a que han aparecido
otros nuevos, como el serotipo 35b.
Se presenta el caso clínico de un lactante de un año de edad, previamente sano y con el esquema nacional
de vacunación completo. El cuadro inició con fiebre e irritabilidad, y en pocos días se agregaron crisis convulsivas
refractarias. Durante la hospitalización desarrolló pancitopenia y sangrado del tubo digestivo alto. A pesar del
manejo de soporte, tuvo un desenlace fatal al sexto día del inicio de los síntomas. El análisis post mortem del
líquido cefalorraquídeo confirmó meningitis bacteriana por Streptococcus pneumoniae serotipo 35b. Se resalta la
relevancia clínica y epidemiológica de serotipos emergentes no incluidos en las vacunas del esquema nacional.
