Streptococcus pneumoniae serotype 35b meningitis in a vaccinated infant. Case repor

ABSTRACT
Bacterial meningitis is a major cause of morbidity and mortality in children under five years of age. Streptococcus
pneumoniae is the main causative agent; its epidemiology has changed following the introduction of conjugate
vaccines (pcv10 and pcv13). Serotype replacement has occurred, with new ones emerging, such as serotype 35b.
We present the case of a one-year-old male infant, previously healthy and fully up-to-date with the national
vaccination schedule. The illness began with fever and irritability, and within a few days, refractory seizures developed.
During hospitalization, he developed pancytopenia and upper gastrointestinal bleeding. Despite supportive
care, he died on the sixth day after the onset of symptoms. Postmortem cerebrospinal fluid analysis confirmed bacterial
meningitis caused by Streptococcus pneumoniae serotype 35b. This highlights the clinical and epidemiological
relevance of emerging serotypes not included in the national vaccination schedule.
