Valor predictivo de pruebas de coagulación para requerimientos transfusionales en pacientes críticos: cohorte retrospectiva

RESUMEN

Introducción: la transfusión de plasma fresco congelado (PFC) y crioprecipitados es común en pacientes críticos, aunque las pruebas de coagulación convencionales como INR, TTPa y plaquetas tienen uso frecuente, este estudio evaluó su capacidad para anticipar requerimientos transfusionales. Materia y métodos: se realizó una cohorte retrospectiva con 526 pacientes adultos ingresados en la Unidad de Cuidados Intensivos (enero 2024-mayo 2025). Tras aplicar criterios de exclusión y depurar registros incompletos, 314 pacientes conformaron la cohorte final. El desenlace primario fue la transfusión de PFC o crioprecipitado en las primeras 48 horas. Se construyó un modelo de regresión logística con validación interna mediante bootstrapping (1,000 réplicas), evaluando discriminación (AUC) y calibración (Hosmer-Lemeshow). Resultados: de 314 pacientes con datos completos, 45 (14.3%) recibieron transfusión. El único predictor independiente fue el sangrado activo (OR 6.27, IC95% 2.47-15.93, p < 0.001). INR y plaquetas no fueron significativos. El modelo mostró AUC 0.69 (IC95% 0.60-0.77) y calibración adecuada (p = 0.17). Conclusiones: las pruebas de coagulación convencionales tuvieron utilidad limitada para guiar transfusión; sólo el sangrado activo fue clínicamente relevante. Se apoya un enfoque restrictivo y la necesidad de pruebas hemostáticas funcionales más precisas.
