Manejo del tétanos en cuidados intensivos: revisión actualizada de fisiopatología, terapias dirigidas y soporte en la UCI

RESUMEN

Introducción: el tétanos severo continúa siendo un desafío relevante en unidades de cuidados intensivos (UCI), especialmente en regiones de Latinoamérica donde persisten brechas de vacunación y diagnósticos tardíos. Los pacientes críticamente enfermos suelen presentar espasmos refractarios, disfunción autonómica y complicaciones respiratorias que requieren soporte avanzado. Objetivo: revisar la evidencia actual sobre fisiopatología, criterios de gravedad, diagnóstico y manejo integral del tétanos en la UCI, enfatizando estrategias terapéuticas actualizadas y prevención de complicaciones. Material y métodos: se realizó una revisión narrativa de la literatura reciente (2010-2024), con énfasis en estudios clínicos, cohortes de la UCI, guías de manejo y ensayos que evaluaron terapias dirigidas a espasmos, disautonomía y soporte vital. Se incluyeron únicamente estudios en población adulta. Resultados: la fisiopatología mediada por tetanospasmina explica la rigidez, espasmos y la inestabilidad autonómica que condicionan la necesidad de ventilación mecánica y manejo hemodinámico estrecho. La administración temprana de inmunoglobulina antitetánica, el control escalonado de espasmos con benzodiacepinas y el uso selectivo de relajantes neuromusculares constituyen los pilares terapéuticos. La disautonomía requiere monitorización invasiva y manejo farmacológico preciso. Las complicaciones frecuentes incluyen rabdomiólisis, arritmias, neumonía asociada con ventilador y sepsis. La rehabilitación temprana mejora los desenlaces funcionales. Conclusiones: el manejo del tétanos en la UCI debe ser protocolizado, priorizando la neutralización inmediata de la toxina, el control eficaz de espasmos y la estabilización autonómica. La prevención de complicaciones y la rehabilitación temprana son fundamentales para mejorar la supervivencia y la recuperación funcional. Persisten vacíos de evidencia sobre terapias autonómicas, uso de magnesio y protocolos estándar de sedación, lo que demanda investigación adicional.
