Cada año millones de personas en el mundo viven y mueren con sufrimiento evitable relacionado con la salud, debido al acceso limitado a cuidados paliativos. En este contexto, la pregunta de «morir en casa o en el hospital» suele plantearse como dicotomía principalmente geográfica o incluso emocional; sin embargo, el lugar donde ocurre la muerte no es únicamente el resultado de una preferencia individual, sino el reflejo de procesos clínicos, decisiones anticipadas, organización del sistema de salud, acceso a recursos, respeto a los principios y valores individuales. En este sentido, el lugar de muerte se convierte en un indicador indirecto del desarrollo de los cuidados paliativos.
Las trayectorias hacia la muerte varían según el tipo de enfermedad: en algunos casos la muerte súbita, principalmente en enfermedades cardiovasculares; en cáncer avanzado puede observarse un declive progresivo; en enfermedades crónicas avanzadas se presentan exacerbaciones intermitentes con recuperaciones parciales; en enfermedades neurodegenerativas existe deterioro lento y prolongado, asociado con fragilidad y dependencia.
