INTRODUCCIÓN
La valoración pediátrica comienza por entender
que el niño no es un adulto pequeño;
su fisiología impone una ventana de seguridad
mucho más estrecha. La Sociedad Americana de
Anestesiólogos (ASA) indica que el 30% de las
muertes en niños se deben a complicaciones en
el manejo de la vía aérea.
1. Hipoxemia crítica: es la complicación más común
debido a un consumo de oxígeno elevado
(6-8 mL/kg/min vs 3 mL/kg/min en adultos).
2. Dinámica alveolar: la capacidad residual
funcional está disminuida, lo que reduce el
reservorio de oxígeno durante la apnea.
3. Mecánica pulmonar: existe una menor compliance
pulmonar, lo que aumenta el riesgo
de colapso alveolar y dificulta la ventilación
con presión positiva(1).
