Leucemia mieloide crónica: desafíos clínicos en el tratamiento de la resistencia a inhibidores de cinasa de tirosina y complicaciones cardiovasculares

RESUMEN

ANTECEDENTES: La leucemia mieloide crónica ha transformado su pronóstico gracias a la administración de inhibidores de tirosina cinasa. Sin embargo, la aparición de resistencia y complicaciones cardiovasculares continúa siendo un reto, particularmente en América Latina.
CASO CLÍNICO: Paciente masculino de 45 años con leucemia mieloide crónica en fase crónica, portador de la mutación T315I, quien fue tratado con ponatinib tras pérdida de respuesta molecular mayor con imatinib. El paciente alcanzó respuesta molecular profunda (MR4.5), pero tuvo una trombosis venosa profunda que requirió suspensión temporal del fármaco, anticoagulación y ajuste de dosis. La terapia se reanudó a 30 mg diarios, con lo que logró mantener la respuesta sin recurrencia trombótica.
CONCLUSIONES: La experiencia demuestra que es posible lograr un control profundo y sostenido de la enfermedad con un tratamiento integral que equilibre eficacia y seguridad. Este enfoque resulta especialmente relevante en contextos latinoamericanos, donde persisten desafíos en acceso a tratamientos, diagnóstico oportuno y seguimiento molecular.
