Chronic myeloid leukemia: Clinical challenges in the treatment of tyrosine kinase inhibitor resistance and cardiovascular complications

ABSTRACT

BACKGROUND: Chronic myeloid leukemia prognosis has significantly improved with tyrosine kinase inhibitors. However, treatment resistance and cardiovascular complications remain challenges, especially in Latin America.
CLINICAL CASE: A 45-year-old male patient diagnosed with chronic-phase chronic myeloid leukemia carrying the T315I mutation, who was treated with ponatinib after loss of major molecular response on imatinib. The patient achieved a deep molecular response (MR4.5) but developed deep vein thrombosis, prompting temporary treatment suspension, anticoagulation, and dose adjustment. Therapy was resumed at 30 mg daily, maintaining response without recurrence.
CONCLUSIONS: This experience demonstrates that deep and sustained disease control is achievable through comprehensive management, balancing efficacy and safety. Such an approach is especially critical in Latin America, where challenges in access to treatment, timely diagnosis, and molecular follow-up persist.
