Distrofia miotónica congénita tipo 1; la importancia del estudio de la familia

RESUMEN
Introducción: la distrofia miotónica congénita (DMC), es la
forma grave de la distrofia miotónica tipo 1 (DM1), transmitida
generalmente por vía materna, caracterizada por hipotonía e
insuficiencia respiratoria con una elevada mortalidad.
Objetivo: describir los hallazgos clínicos y la genealogía de una
recién nacida con DMC.
Caso clínico: la paciente nació a
las 31 semanas de edad gestacional. Durante el embarazo
tuvo polihidramnios y disminución de movimientos fetales. Al
nacimiento presentó hipotonía generalizada, apnea, requiriendo
reanimación avanzada y dependencia permanente
de ventilación mecánica. Presentó arreflexia, dificultad grave
para su alimentación, episodios recurrentes de broncoespasmo,
sobreinfecciones, falleciendo a los de 73 días de vida.
El estudio molecular del gen DMPK demostró la presencia
de un alelo detectable por PCR, hallazgo coherente con
expansión excesiva del triplete CTG. La genealogía reveló
10 familiares afectados con manifestaciones clínicas de DM1
a diferentes edades, en cuatro generaciones, con un patrón
autosómico dominante.
Conclusión: la DMC es una enfermedad
con un patrón de herencia autosómica dominante,
con fenómeno de anticipación genética. Esta entidad debe
considerarse dentro del diagnóstico diferencial de neonatos
con hipotonía generalizada.
