Tratamiento quirúrgico precoz de las fracturas del gancho del hamate con fijación mediante tornillos sin cabeza: una serie de casos latinoamericanos

RESUMEN
Introducción: el diagnóstico de las
fracturas del gancho del hamate a partir de la sospecha
clínica se puede realizar con radiografías convencionales
y tomografía axial computarizada (TAC). El manejo
incluye opciones como la inmovilización, la resección de la
apófisis y la reducción abierta con fijación interna (RAFI).
El objetivo de este estudio fue explorar si los pacientes
manejados con RAFI recuperan la movilidad de la muñeca
y la fuerza de agarre de la mano.
Material y métodos:
estudio retrospectivo analítico de serie de casos en pacientes
adultos con fractura de la apófisis unciforme, tratados
mediante RAFI con tornillo canulado sin cabeza, entre 2019
y 2021. Se excluyeron pacientes con otras fracturas de la
extremidad superior ipsilateral y contralateral y aquéllos
que no tuvieron seguimiento. Se evaluaron los arcos de
movilidad de la muñeca, fuerza de agarre, escala de dolor
y complicaciones.
Resultados: se incluyeron 10 pacientes,
ocho de sexo masculino. El tiempo de seguimiento
promedio fue de 16.7 meses (rango entre 12 y 25 meses). La
evaluación posoperatoria reveló bajo dolor (EVA promedio:
1.4), mejoría de la función (QuickDASH promedio: 2.7), sin
diferencias estadísticamente significativas al comparar los
promedios de fuerza y arcos de movilidad del lado afectado
con el lado sano. Se presentó una complicación por posición
del tornillo en la articulación carpometacarpiana que causó
dolor, por lo cual fue necesario el retiro del dispositivo.
Conclusión: los pacientes con fracturas del gancho del
hamate manejados con RAFI recuperaron la movilidad de la
muñeca y la fuerza de agarre de la mano afectada.
